Hospital Discharge Follow-up

The transition from hospital to home can be overwhelming. Our hospital discharge follow-up service is designed to support this process and ensure patients continue on the road to recovery in their own home. Through a phone call with our Registered Nurse, a patient’s health status, medications, appointments and emergency readiness will be reviewed. We hope to identify and address concerns from patients, caregivers and family members to prevent readmission to the hospital.
