The goals of this program are to educate and support patients who have been diagnosed with diabetes. A Certified Diabetes Educator (CDE) will meet with patients to teach them how to manage this chronic disease. Other family health team folks will support foot care, nutrition, glucometer training, and getting moving—all components of a successful diabetes management plan.
Also, some patients who are identified as being at risk of becoming diabetic are encouraged to meet with the dietitian to discuss lifestyle changes and the CDE to learn about minimizing the risk.
