Diabetes Management Program

Our Diabetic Management Program of the Portage Medical Family Health Team provides focused support for patients and their families with pre-diabetes and diabetes who are struggling to achieve optimal control of their diabetes. This program will help you live a healthy life with diabetes.  The Team consists of a Nurse Practitioner and Registered Dietitian who works in conjunction with your Family Physician and will talk about healthy food choices, activity, education, medications, blood sugar testing, lab testing and overall quality of life. Patients will be empowered to be self-directed in learning the skills and behaviours needed for successful self-management of their diabetes. Your Family Physician or Nurse Practitioner may recommend that you attend this program if your blood sugar is higher than normal.  You may also attend this program if you would like more information about your pre-diabetes or diabetes or if you would like help managing your diabetes.

Diabetic visits are patient-directed.  A variety of diabetes topics that is important to the long-term management of your diabetes will be discussed.  Our goal is to help give you the tools you need to feel comfortable and confident managing your health day-to-day. 

If you are interested in managing your diabetes, you can ask your Family Physician or nurse practitioner for a referral to the Diabetic Management Program.
